I.
Life Skills Centers, Inc. conducts a complete background review of all new employees. In order to comply with various State and Federal laws and regulations we are also conducting employee background checks on an annual basis. These background reports may include, but are not limited to, a review of criminal records, public court records, public records, licensing and certification records, financial/credit history, education information and driving records. The timing of your annual background check will likely occur on or about your employment anniversary date. As a condition of your continuing employment at Life Skills Centers, Inc., we require your consent to these annual background reviews and other times as may be required. By signing below, you consent and agree to background checks at any time throughout your employment.

I understand and agree that an investigative background report may be generated on me that may include information as to my character, criminal background check, public court records, public records, employment history, professional license or credentials or certification, financial/credit history; education information and driving record history or other Department of Motor Vehicle (DMV) information. I understand that Life Skills Centers, Inc. or its authorized agents may be requesting information from public and private sources about any of the information noted earlier in this paragraph for use in connection with my employment now, or at any time during my tenure with Life Skills Centers, Inc. and give my full consent for this information to be obtained and used by Life Skills Centers, Inc. or its authorized agents.

I do, for myself, my heirs, executors and administrators, hereby release and discharge and agree to hold harmless Life Skills Centers, Inc. and its agents and employees from and against any and all claims, damages and losses, including reasonable attorney fees, in any way arising out of or by reason of Life Skills Centers, Inc. obtaining and/or using my background information and the use of the information derived from a background check, including, but not limited to, the making of any adverse employment decision.
IV.
I understand that the information obtained by Life Skills Centers, Inc. through the background check will be used for employment purposes, but that a criminal offense will not necessarily preclude me from continued employed or engagement with Life Skills Centers, Inc. All such information will be used to evaluate suitability for employment or continued employment based on a variety of circumstances, including the nature of the crime, recentness of the crime, and the type of position held and in accordance with applicable law.

I acknowledge that a copy of this release shall be as valid as the original.

I hereby authorize, without reservation, any financial institution, law enforcement agency, information service bureau, school, employer or insurance company contacted by Life Skills Centers, Inc. or our authorized agents, to furnish the information described in this consent and authorization form.
